MADISON ANIMAL HOSPITAL
ALLIED HEALTH PROGRAM
MEMORANDUM OF AGREEMENT

THIS AGREEMENT, made this_ 3‘ day of Feb, 2023, by and
between, MADISON ANIMAL HOSPITAL, hereafter referred to as the Health Agency and BOONE
‘CAREER and TECHINICAL CENTER hereafter referred to as the Boone Center.

WHEREAS, the Health Agency and the Boone Center have agreed to
jointly participate in a program in which students from the Boone Center's Allied Health
Programs may acquire experience in the Health Agency.

NOW, THEREFORE, the parties agree as follows:
1. This agreement may be terminated by either party within thirty (30) days written notice.

2. The program will be jointly evaluated from time to time and amendments may be made to
this agreement. Any amendments will be reduced to written memorandum and attached
hereto.

3. Neither monetary considerations nor any other benefits will be provided to the students
assigned to the Heaith Agency.

4. This agreement and the Implementation thereof shall be in accordance with all the non-
discriminatory principals and reguiations of the federai government as administered by the
Office of Equal Opportunity and other such agencies.

5. The number of students assigned to the Health Agency will not exceed two (2) health
occupations students.

6. ‘The students understand that due to occupational exposure to blood or other infectious
materiais they may be at risk of acquiring hepatitis B virus (HBV) infection of human
immunodeficiency virus (HIV} infection, which causes aids. The students were told to be
vaccinated against hepatitis by taking the Hepatitis 8 vaccine at their own cost. However, if
they decline, they understand that by declining this veccine they continue to be at risk of
acquiring any infectious disease while in e clinical situation. The students have been
instructed in the use and requirement for this protective equipment as well as ail standard
precautions, including hand washing techniques, disposal of infectious wastes, the HIPPA
laws, ethics, and confidentiality of all infarmation they have learned about patients.

The Soone Center agrees:
1. To submit goats and objectives for each student rotation.
2. To provide all didactic instructions to the students.

3. That the student will be officially registered at the Boone Center in the Allied Health
Programs.

4. To provide the Health Agency with evidence that each student is covered with malpractice
insurance.

5. To provide the Health Agency with evidence that each student has passed a recent physical
examination before beginning clinical rotation.

& Te require students to observe and obey all rules, reguiations, policies and procedures of the
Health Agency.

7. Students will be subject to testing for drugs of abuse prier to entering the clinical facility. the
school may conduct random drug tests periodically during the students’ clinical experiences.

The Health Agency agrees:

1. To provide adequate facilities to carry out the program including clinical facilities available; to
include laboratory, examination raoms, reception desk, businass office and/or home visits.

2. To help plan and implement meaningful and appropriate learning experience aimed at the
achievement of the objectives of clinical preparation.
