period of April 16 through May 30, 2024, in the amount of $1,233,189.00.

The Superintendent recommends approval of the contract with Kathy Ramsey to

The Superintendent recommends approval of the quote from Snap-On Industrial

with VIN Scanner.

2598 CONTRACT —- DR. ELISE MOORE —- FUNCTIONAL VOCATIONAL
ASSESSMENTS
The Superintendent recommends approval of the Contract with Dr. Elise Moore
to provide Functional Vocational Assessment services for the 2024-2025 SY, in
the amount of $100.00 per evaluation report and $50 per IEP meeting including
in county travel reimbursement.

2599 CONTRACT —- SIGN WORKS SERVICES —- SUBSTITUTE INTERPRETER
The Superintendent recommends approval of the Interpreter/Consulting
Services Contract with Sign Works Services to provide substitute sign language
interpretation for the 2024-2025 SY, contract not to exceed
$21,600.00.

2600 CONTRACT — MILESTONES & MUSIC, LLC - MUSIC THERAPY SERVICES
The Superintendent recommends approval of the contract with Milestones &
Music, LLC for the 2024-2025 SY, at the rate of $650.00 per SEMTAP
Assessment, $160.00 per hour for direct service, consult services, and
educational enrichment service, $250 per hour for direct service provided for 2
students simultaneously, $300 per hour for direct service provided for 3
students simultaneously, $100 per hour for meetings, $100-$200 per hour for
presentations / in-services / professional development, and $50 for 30 minutes

to review expectations with case manager. Not to exceed $100,000.00.

2601 CONTRACT —- LORI BAILEY —- PSYCHOLOGY EVALUATIONS
The Superintendent recommends approval of the contract with Lori Bailey to
provide Psychology Evaluations for the 2024-2025 School Year. The rate of
services is $350 per evaluation, $50 per Supplemental data, $200 per cognitive
evaluation, $150 per achievement evaluation, $250 per ADOS-2/Autism
Evaluation (standalone evaluation) and $150 per ADOS-2/Autism Evaluation as
supplemental data. Not to exceed 250 evaluations.

The Superintendent recommends approval of the contract with Lisa Stafford to
provide Psychology Evaluations for the 2024-2025 School Year. The rate of

evaluation, $150 per achievement evaluation, $250 per ADOS-2/Autism

2606 CONTRACT —- GIA DEASY — CONTRACTED SERVICES
The Superintendent recommends approval of the agreement with Gia Deasy to
provide contracted services for observations for the 2024-2025 SY. The rate of
services is $30 per thirty (30) minute student observation and $15 per each
additional fifteen (15) minute interval thereafter. Not to exceed 500 hours.

2607 CONTRACT —- JANIE GALLAGHER — CONTRACTED SERVICES
The Superintendent recommends approval of the agreement with Janie
Gallagher to provide contracted services for observations for the 2024-2025 SY.
The rate of services is $30 per thirty (30) minute student observation and $15
per each additional fifteen (15) minute interval thereafter. Not to exceed 500
hours.

2608 CONTRACT —- CHRISTINE HOLSOPPLE — CONSULTATIVE SERVICES
The Superintendent recommends approval of the agreement with Christine
Holsopple to provide consultative services to the pupil services department in
the areas of IEP training sessions, professional development on IEP writing and
compliance, WVEIS 2.0 IEP platform for report retrieval and WVDE submissions,
consultation with liaisons regarding IEP compliance, training on electronic filing
systems, and DLM training. The rate of services is $50 per hour. Not to exceed
200 hours.

2609 CONTRACT —- ROSA ANNA GALLUCCI —- FUNCTIONAL VOCATIONAL
ASSESSMENTS
The Superintendent recommends approval of the contract with Rosa Anna
Gallucci to provide Functional Vocational Assessment services for the 2024-2025
SY, in the amount of $50 per hour. Not to exceed 20 hours.

2610 N2Y, LLC —- MATH MANIPULATIVES
The Superintendent recommends approval of the purchase of the Math
Manipulatives and Paper Kit, a hands on learning that strengthens math skills,
improves critical thinking in students and builds on their conceptual
understanding for the 2024-2025 SY, in the amount of $123,999.60.

2611 N2Y, LLC —- RENEWAL OF UNIQUE LEARNING SYSTEM
The Superintendent recommends approval of the renewal of Unique Learning
System, for the 2024-2025 SY, in the amount of $31,467.17.

2612 EXPLORELEARNING —- REFLEX MATH SUBSCRIPTION
The Superintendent recommends approval of the purchase of a one-year
subscription for the 2024-2025 SY, for 455 students (with two trainings
included), in the amount of $14,332.50.

2613 READING HORIZONS - ONLINE SOFTWARE
The Superintendent recommends approval of the quote from Reading Horizons
to purchase an online software program including unlimited site access to the
Reading Horizons Professional Development Course, for the 2024-2025 SY, in
the amount of $37,400.00.

2614 HOUGHTON MIFFLIN HARCOURT - LITERACY R180 - RENEWAL
The Superintendent recommends approval of the subscription renewal of
Literacy R180 U Stage C for the 2024-2025 SY, in the amount of $10,900.00.

2615 TEACHTOWN —- SUBSCRIPTION RENEWAL
The Superintendent recommends approval of the subscription renewal of
TeachTown ecCORE K-12 Student Subscription Package for the 2024-2025 SY,

2616 PSNI - SNAP HEALTH CENTER - SOFTWARE PROGRAM
The Superintendent recommends approval to purchase a new software program,
SNAP Health Center, for the 2024-2025 SY, in the amount of $28,000.00.

immunization system and Medicaid export was an additional cost.)

2617 MOU —- WVU BOARD OF GOVERNORS - CVRP
The Superintendent recommends approval of the Memorandum of
Understanding for WVU Board of Governors and its School of Medicine,
Department of Ophthalmology and Visual Sciences to provide vision
rehabilitation services to blind and visually impaired school-age students for the
2024-2025 School Year. The Orientation and Mobility Specialist will be
comparable to the WVDE Professional Salary Schedule based on years of
experience and education plus $18,600.

2618 MOU - WVU (WVUSM) - STUDENT CLINICAL
The Superintendent recommends approval of the MOU Student Affiliation
Agreement with West Virginia University and its School of Medicine, Department
of Human Performance and Applied Exercise Science to permit students to
participate in the fieldwork of Occupational Therapy and/or Physical Therapy for
the 2024-2025 SY.

2619 CONTRACT —- BEST LIFE THERAPY —- SUMMER PROGRAMS
The Superintendent recommends approval of the Contract with Best Life
Therapy to provide sign language interpretation services, in the amount of
$50.00 per hour for the 2024-2025 extended SY. The terms of this contract is
not to exceed $10,000.00.

The Superintendent recommends approval of the Contract with Best Life
